"Organizational disasters occur as a result of ineffective decision making." Is this a valid claim?

What other examples are there other than the 'Challenger disaster'?

  1. dtjessb: I believe that org. disasters occur more often because of lack of communication and the culture of the organization. The Challenger happened because the group had not developed a culture where everyone's point of view was important. As for Katrina, I believe the fact that communication in the government takes forever to get a message up the ladder and then get a decision to act down it again. I would say so. Take a look at Katrina, I can't think of a bigger organizational disaster, can you? So many people died because the organization wasn't qualified or trained properly, and this happens all the time.
To answer your question in one word, FEMA.

other examples can be pointed out around Technological Disasters. Look at Yahoo!'s recent blunders, look at Microsoft's.
