You fill out your state's forms and file an application fee.

For Kentucky, you can find the forms by loading the following URL: http://sos.ky.gov/business/filings/forms/

Look for "Domestic Limited Liability Companies Forms" which has 3 form options: KLC, Professional Limited Liability Company (PLC), NLC

Likely you'll want to use the KLC.

If you're a non-profit use the Non-profit LLC (NLC)

If you're a doctor, chiropractor, lawyer, accountant, architect or engineer, you'll use the PLC form.

And if you are neither a NLC or PLC use the standard KLC form.
